Funds will be used to purchase aluminum ramps for terminally ill people referred by Oswego County Hospice, making it possible for those individuals to remain at home with their families and continue to have access to the community.
Home Access For All: Home Access for All is a two-part program. It will use the CAPS (Certified Aging-in-Place Specialist) assessment process to educate the community, perform assessments, and make it possible for people to prevent falls, remain safely at home or return from a hospital, rehab center or nursing home.
